We provide experienced, responsive, and high-quality in-home medical care for those in need.

About the Program

The Whole Person provides skilled medical professionals, including Registered Nurses, Licensed Practical Nurses, Clinical Social Workers, Physical Therapists, Occupational Therapists, Speech-Language Pathologists, and Certified Wound Nurses, to address acute illness, post-injury, or post-surgical needs.

Medical Health Assessment

We provide a free in-home assessment to determine eligibility and collaborate with your physician for necessary care orders.

Requirements

Provided Services

Skilled Nursing

Our nurses provide comprehensive care, including:
• Post-hospital care
• Patient education
• Medication management
• IV therapy
• Health issue management (e.g., diabetes, CHF, lung disease)

Wound, Ostomy, and Continence Nursing

Specialized care includes:
• Skin breakdown prevention
• Level II Wound Vac Certified staff
• Specialty dressings and post-surgical care
• Certified Wound Nurse support

Therapy Services

• Physical Therapy: Restores mobility, strength, and endurance.
• Occupational Therapy: Helps regain daily living skills post-injury or illness.
• Speech Therapy: Treats communication, swallowing issues, and facial nerve pain.

Medical Social Services

Our social workers assist with:
• Health-related counseling for individuals and families
• Accessing community resources
• Long-term planning and advanced directives/living wills
