Transition Services

The Show-Me Home team will work with you every step of the way, from the planning process to finding housing and setting up your new home.

About the Program

The Show-Me Home program helps Missourians aged 18 and older with physical disabilities, transition from nursing homes to independent community living. We offer personalized support, housing options, and essential services to make the move successful. Our team guides participants through planning, finding housing, applying for support, and settling into their new home.

Am I Eligible for the Show-Me Home program?

Participants receive support for one year and continue with home and community-based services through MO HealthNet if eligible.

To receive help through Show-Me Home, you must meet the eligibility:

Requirements

Live in Missouri
Have lived in a nursing facility for at least 60 days
Have a physical disability or be at least 18 years old
Be eligible for or currently have Missouri Medicaid (MO HealthNet) before moving out of a nursing facility
Must have a source of income
Able to move into a home that is leased or owned by you or a family member, or move to residential housing with no more than four people living in the house

Get Started with Show-Me Home

To find out if you qualify for Show-Me Home, the nursing home resident needs to speak to their social worker and request a referral. You can always contact us if you have questions.

Transition Services for Kansas Residents

Transition services for Kansas residents must be referred through Healthy Blue or Sunflower Managed Care.

Youth Transition Services

Develop life skills for children and youth (ages 14-21) to transition toward independence and adulthood.

Current Programs

High School Youth Transition to Independence Program

An 8-week high school course that provides students with skills and resources necessary to have for transitioning from high school into adulthood. Contact our team for more information about this program.

Virtual Transition Classes

Parents & Teachers – The Whole Person would like to connect with you and your student through “Virtual” Transition Classes! Our classes are designed to provide enlightening topics that will prepare students with disabilities for life after High School.

Topics such as:

Introduction, Diversity & Disability
Disability Awareness & Accommodations
People First Language
Advocacy & Support Systems
Communication
Employment
Stress Management & Problem Solving
