Transition Services
About the Program
The Show-Me Home program helps Missourians aged 18 and older with physical disabilities, transition from nursing homes to independent community living. We offer personalized support, housing options, and essential services to make the move successful. Our team guides participants through planning, finding housing, applying for support, and settling into their new home.
Am I Eligible?
Participants receive support for one year and continue with home and community-based services through MO HealthNet if eligible.
To receive help through Show-Me Home, you must meet the eligibility:
Requirements
- Live in Missouri
- Have lived in a nursing facility for at least 60 days
- Have a physical disability or be at least 18 years old
- Be eligible for or currently have Missouri Medicaid (MO HealthNet) before moving out of a nursing facility
- Must have a source of income
- Able to move into a home that is leased or owned by you or a family member, or move to residential housing with no more than four people living in the house

Transition Services for Kansas Residents
Transition services for Kansas residents must be referred through Healthy Blue or Sunflower Managed Care.
